There are different attribute types that can be used to gather information from within a Form.
You can use the below definitions to assist in your choice:
Text: A text input field limited to a maximum of 150 characters
Text Multiline: A text input field limited to a maximum of 400 characters
Text Multiline: A text input field limited to a maximum of 400 characters
Text (Encrypted): A text input field limited to a maximum of 150 characters. These are encrypted after entry. See article Can I collect sensitive information from my users?
Option (Select one): Create a list of options in dropdown format in which the end user can select one option.
Option (Select multiple): You can create a list of options for multiple choice selection.
Radio Button: Create a multiple choice list where the end user can only select one option. If you want users to be able to multi-select, you the checkbox question type.
File (Document): Prompt your end users to upload a .doc,.docx,.odt,.pdf,.rtf,.txt file type. (Maximum 16mb file size)
File (Image): Upload .png, .gif, .jpg,.jpeg, .tif type only.(Maximum 16mb file size)
File (Document/Image): Prompt your end users to upload a file in line with the above formats. (Maximum 16mb file size)

Date/Time Select: Typically used to add expiry dates or collect date of births, add a date select question to provide a dropdown for day, month and year.
Yes/No : A simple booleon slider with the option for the end user to select either 'left for Yes and right for No.
Country Dropdown: List of countries in alphabetical order. By default, a State dropdown menu will appear below this question
